Submission Requirements
- ASIS Refuse Haulers Application
- Vehicle List Spreadsheet to include: Year, Make, Model, GVW, Cost New
- Property list spreadsheet to include: Cope Information, Statement of Values
- Currently valued loss runs for a minimum of four (4) years
- Current MVR's for all drivers
- Short narrative on operations and loss control procedures
Claims Management
- GBS is the claims administrator for this program

Strategic HR provides value added loss control services on request. Surveys available are:
